Summary
Background
Monkeypox virus clade I is endemic in several central African countries and characterised by an increase in disease severity and mortality. Since October, 2023, a large-scale mpox outbreak has emerged in DR Congo, and in March, 2024, the first individuals with mpox were reported outside the endemic areas in Republic of the Congo. We aimed to provide insight into the epidemic by sequencing samples obtained from individuals with mpox in Republic of the Congo.
Methods
In this cross-sectional, descriptive study, samples were collected from individuals with suspected mpox between Jan 15 and April 8, 2024, in Brazzaville, Pointe-Noire, Likouala, Cuvette-Centrale, and Plateaux (Republic of the Congo). Blood samples, skin or oropharyngeal swabs, or skin crusts were obtained for molecular diagnosis via real-time PCR. Monkeypox virus sequences were obtained and analysed using newly established nanopore sequencing methodology and bioinformatic pipeline. The sequences obtained were aligned and used to construct a maximum likelihood phylogenetic tree using IG-TREE.
Findings
61 samples were collected from individuals with suspected mpox, 31 of which were positive for monkeypox virus and were included in our analysis (four positive samples were excluded due to unavailability of epidemiological data or insufficient biological material). Individuals who tested positive for monkeypox virus were from Cuvette-Centrale (19 [61%] of 31), Likouala (eight [26%]), and Pointe-Noire (four [13%]). 20 (65%) were male and 11 (35%) were female. Phylogenetic analysis of sequences showed two major clusters within clade Ia. One cluster was made up of four sequences from this study clustering with two monkeypox virus sequences from the current DR Congo outbreak, three older sequences from Central African Republic sequenced between 2017 and 2018, and seven sequences from DR Congo sequenced in 2006–07 and 2022. The second cluster was made up of 16 sequences from this study clustering with sequences from the current DR Congo outbreak. In addition, sequences from Republic of the Congo show multiple phylogenetic positioning suggesting the occurrence of multiple co-circulating strains in the human population.
Interpretation
Our findings suggest that multiple monkeypox virus strains are co-circulating in the human population, highlighting the need for implementation of expanded mpox surveillance, especially in countries bordering DR Congo and Republic of the Congo, in combination with control measures focused on containing the current outbreaks in these countries to prevent escalation into a larger-scale epidemic.

Source: UKHSA, https://www.gov.uk/government/news/ukhsa-detects-first-case-of-clade-ib-mpox
Two cases of Clade Ib mpox have been detected in household contacts of the first case, the UK Health Security Agency (UKSHA) can confirm. This brings the total number of confirmed cases to 3.
The 2 patients are currently under specialist care at Guy’s and St Thomas’ NHS Foundation Trust in London. The risk to the UK population remains low.
There has been extensive planning underway to ensure healthcare professionals are equipped and prepared to respond to any further confirmed cases.
Professor Susan Hopkins, Chief Medical Adviser at UKHSA, said:
”Mpox is very infectious in households with close contact and so it is not unexpected to see further cases within the same household.
”The overall risk to the UK population remains low. We are working with partners to make sure all contacts of the cases are identified and contacted to reduce the risk of further spread.
”Contacts of all 3 cases are being followed up by UKHSA and partner organisations. All contacts will be offered testing and vaccination as needed and advised on any necessary further care if they have symptoms or test positive.

Despite the availability of an effective vaccine — which costs about $100 a shot — and countries like the US having stockpiled millions of doses, Congo has yet to receive any. WHO delays in authorizing vaccines have prevented shots from reaching needy countries as quickly as they could have, say critics.

London currently has the most Mpox cases in England
www.mylondon.newsOn August 14, the World Health Organization declared a growing outbreak of #mpox in the Democratic Republic of the Congo (DRC) and other African countries a #global #health #emergency of international concern.
The alert marks the second time in three years that an international health emergency has been declared due to the rapid spread of the disease formerly known as #monkeypox.
The last mpox emergency designation was in July 2022, when an outbreak extended across 116 countries, impacting nearly 100,000 people, and killing about 200.
In the latest outbreak, more than 15,000 confirmed and suspected cases of mpox have been reported in Central and Western Africa along with 537 deaths
-- making the new outbreak deadlier than the one two years ago.

and she led a team that developed novel genome sequencing approaches for viruses such as mpox to better investigate how they emerge and spread.

Scientific European»Much has been made of the fact that many of the initial diagnoses were in sexual health clinics by gay men, not because it is primarily sexually transmitted, like HIV, but because gay men are much more attuned to unusual health issues being indicative, and are more likely to seek treatment than straight men.
In the US, this has led to women and straight men being misdiagnosed, and risks being misanalyzed. The emphasis has been on not having sex, particularly gay sex for the right. So that’s one concerning element of this - I am sure that the scripts are already being written on the right.

And it is as yet impossible to rule out the MOST concerning issue - whether the rise of monkeypox, and its rather rapid mutation in both Clades, is in any way linked to immune damage due to Covid.
That’s speculation, but a lot of people are concerned. You all remember Kaposi’s Sarcoma, a kind of skin cancer whose sudden rise to prominence was one of the things that led to the identification of the HIV virus and AIDS, right?
Sometimes, the arrival of a new disease or a disease in a new population indicates some other underlying condition. For example, we know that measles makes it more likely for up to five years that children will die of other infections. The post-viral wiping of the immune system increases the risk of death overall.
Post viral immune damage from covid means that the combination of widespread, repeat infection with covid and an immune damaged population exposed to a new to them pox virus may have more terrible consequences than previously. If so, Covid could open the door to new kinds of infections and viruses that had not been a previous problem for most of us.
Pox viruses also have immune implications, for they increase the quantity of NK (natural killer cells), but turn off their function, making them ineffective, at least in animal studies. We know that Covid also damages and causes dysfunction in NK cells, and that the two might harmonize in some troubling ways.
This last is the biggest worry I have - not that Mpox will be as prevalent as covid, but that mpox will become common in places where children have the most physical contact, and that it will have a high mortality rate due to its inherent severity in kids and possible immune dysfunction due to Covid from our high levels of transmission.
The other major concern I have is that because monkeypox takes three full weeks to heal AFTER LESIONS form, and one is highly contagious during that time, that unless we get ahead of this very soon, and provide economic support and isolation facilities for people affected, mpox could get away from us, simply because most people cannot afford to take that much time off work.«
