What is a **lightweight** piece of wiki software that can be configured so that any changes must be approved before they are applied?

MediaWiki is entirely too much for my purposes, I just want self-hosted collaborative text editing with formatting and ToC on which I can control whether changes are added.

@jessmahler I'd probably try to do some sort of PHP/javascript front-end that uses the git version/commit control method at the back end.... which I can already hear developers screaming about in horror in the distance
