Lay people may understand it as: "high probability of (twin) earthquakes [in the near future] in Nankai Trough."
While your message is rather that: IF one M8+ #earthquake happens in the Nankai Trough, THEN the probability of a second big one there is quite high (but with large uncertainty). Japan Times article rightly explain this, but their title not really.
@RobinLacassin I agree with you. I guess they have various restrictions such as the length (in addition to the efforts to attract readers to click on the news). I understand (at least in Japan) that the person who determines the title differs from the journalist who wrote the article, and that often causes problems.
