Kaliningrad is not completely blocked via rail. It's only for shipments of sanctioned items.

Putin knows that any move against Lithuania means WWIII, so it's highly unlikely he'll do anything provocative against a NATO member.

They also still have full access via the Baltic Sea. If they try anything, they will immediately lose that sea port (and eventually all of Russia itself).
