I have one in my Linux system desktop, the motherboard didn’t have the slot so I bought an adapter made it works. But boot the OS from it I still don’t know how. Online find some resource about Windows OS.

@Sphinx Yup, that's what I was doing when I got that Sense readings.

@Sphinx Yeah, I've done a few things, but the I think I am just going to believe this:
[sda] Sense Key : Hardware Error [current]
[sda] ASC=0x44 <<vendor>>ASCQ=0x81
... and break it in half to dispose of it. I just wanted to make sure I wasn't doing something dumb, first. (Like 'Did you press the reset button behind next to the screw at the top?')
