My observations were that some people would "call in sick." And on Monday, they'd get a few comments about, "Oh, I see that you were really sick." possibly with a wink.
And really, no one cared.
There was no keeping it a secret.
But no one really cared.
But hey; we were in California; a generally laid-back place.
Like sandals, shorts and T-shirt to work, programming computers. And maybe some personal car repair in the back, too.

I grew up, through college, in the Los Angeles, California basin. And one thing about taking taking a day or two "off sick" (in the winter, when the local mountains have good snow), is that the tan lines around the ski goggles are unique and easily recognizable. 😆
