Pet peeve: NHS trusts (like the one I'm under) who create their own unique* identification numbers for patients, despite one which already exists and which NHS England (and in some areas, the law) requiring them to use it and describing it as 'fundamental to patient safety'.

* given the state of their IT systems, I doubt they are unique (each patient can definitely have more than one number)
