does anyone struggle with keeping track of chores and self-care?

I've found it difficult to manage anything that is anything beyond daily or weekly. every other day? every 5 days? forget it. I can do it for a while but long term i get the schedule all jumbled.

Definitely. I've found that I need to track those externally, or they'll get left by the wayside regardless of how important and impactful they are.
I used to use tools like HabitRPG (now called Habitica I think), now I just use a todo.txt file with these chores and self-care habits as recurring items. It becomes a fun game for the completionist part of me, to have them all checkmarked before they go red.
