@garyackerman A favorite activity of mine is to ask questions in such situations, deeper and deepr until we can reach the point of "a human made an incorrect assumption" because those are the assumptions I want to learn from, and to teach myself to recognize when I make the same ones.
“The tech broke” is still the most commonly given excuse for folks not doing their job.
