Toothbrush holders, almost invariably, have holes for multiple brushes at the top and then no division whatsoever at the bottom. This means the brushes lean over at all angles, often entangling with each other so they're hard to extract.

Also, they are exposed to the air, which is not sanitary, particularly when they are located in or near a restroom with a toilet.
